Renal Allograft Biopsy Findings in Live-related Renal Transplant Recipients

OBJECTIVE  To determine the pattern of histopathology in living-related, kidney transplant recipients (KTRs) from a centre Khyber Pakhtunkhwa (KPK), Pakistan. STUDY DESIGN Descriptive, observational study. PLACE AND DURATION OF Institute Kidney Diseases, Peshawar, August 2008 to July 2018. METHODOLOGY A retrospective review graft biopsy reports and clinical charts was carried out. Allograft biopsies were done for dysfunction with no apparent cause. The pathology classified according updated Banff classifications. descriptive statistics used tabulate results. RESULTS Out 55 biopsies, 51 (92.73%) males mean age 34.35±9.40 years. 52 percutaneous 10 (19.23%) belonged normal category. Category 2 (borderline rejection) 3 (acute/active cellular seen three (5.7%) one (1.9%) cases, respectively. Interstitial fibrosis/tubular atrophy (Banff 5) observed 18 (34.62%) cases. 6 (others) 19 (36.5%) which calcineurin inhibitors (CNI) toxicity commonest (17 [89.4%] cases). Mixed lesions found mixed category 12 (63.16%) showed both 5 changes most cases showing mild moderate IF/TA; while case had severe IF/TA. Three nephrectomies done, each recurrent oxalosis, nephroblastoma fungal infection. CONCLUSION Among studied specimens, predominant findings, followed by others (mostly CNI toxicity) IFTA categories. frequency acute/active rejections low that chronic higher, keeping delayed biopsies.
